Legacy Health Foundation bridges the gap between seniors and the comprehensive care they deserve. Our evidence-based programs in chronic disease self-management, fall prevention, Medicare navigation, and caregiver support have served over twelve thousand individuals across the Portland metropolitan area since our founding in 2008. Every program is guided by clinical expertise, cultural sensitivity, and an unwavering commitment to health equity.

Chronic Disease Self-Management

Our flagship Stanford-licensed six-week workshop equips participants with practical tools for managing diabetes, hypertension, arthritis, heart disease, and other ongoing health conditions. Led by trained peer facilitators who understand the daily realities of chronic illness, each session builds skills in action planning, symptom management, communication with providers, and emotional wellness.

Comprehensive Elder Wellness Services

From chronic disease management to caregiver respite, our programs address the full spectrum of elder health needs. Every service is designed with input from clinicians, community health workers, and the seniors we serve.

Chronic Disease Self-Management

Stanford-licensed six-week workshop series covering action planning, medication adherence, symptom monitoring, provider communication, and emotional resilience for individuals living with diabetes, hypertension, arthritis, COPD, and heart disease. Peer-facilitated groups of eight to twelve participants meet weekly in community centers, libraries, and faith-based settings throughout Multnomah County.

Fall Prevention & Mobility

Evidence-based balance and strength training sessions designed to reduce fall risk among adults aged 65 and older. Our certified exercise physiologists lead small-group classes incorporating the Otago Exercise Programme and Tai Chi for Arthritis protocols, supplemented by in-home safety assessments identifying hazards such as loose rugs, inadequate lighting, and bathroom accessibility barriers.

Caregiver Support & Respite

Monthly peer support groups and quarterly respite weekends give family caregivers the resources and breathing room they need. Our caregiver navigators connect families with local respite providers, guide Medicaid waiver applications, and offer one-on-one coaching in caregiver stress management, boundary setting, and long-term care planning. Over seventy percent of participants report reduced burnout within three months.
