Medicare Navigations

Individuals received one-on-one Medicare plan comparison and enrollment assistance, resulting in an estimated $1.2 million in aggregate out-of-pocket savings through optimized plan selection and benefits counseling.

CDSMP Graduates

Participants completed the full six-week Chronic Disease Self-Management Program. Post-program surveys showed a 31% improvement in self-efficacy scores and a mean A1C reduction of 0.8 points among diabetic participants within six months.

Home Safety Assessments

In-home fall risk evaluations conducted by trained community health workers. The average home assessment identified 4.7 environmental hazards, with 89% of recommended modifications completed within 90 days of the report.

Total hours of respite care and caregiver support group facilitation delivered this fiscal year. Seventy-two percent of caregiver participants reported reduced burnout and improved coping skills at the six-month mark.

Memory wellness cognitive screenings administered using the Montreal Cognitive Assessment and SLUMS protocols. Forty-one individuals were connected with OHSU’s Layton Aging Center for follow-up evaluation.

Funding & Financial Stewardship

Legacy Health Foundation operates on an annual budget of approximately $1.4 million, sourced from a diversified mix of federal and state grants (42%), individual and corporate donations (31%), foundation grants (18%), and earned program revenue from contracted services with health systems (9%). Our administrative overhead ratio has remained below 14% for five consecutive years, ensuring that the vast majority of every donated dollar directly supports participant-facing programming and services.

Key funders include the Oregon Community Foundation, Meyer Memorial Trust, the Collins Foundation, Multnomah County Area Agency on Aging, the Administration for Community Living, and Providence Health & Services Community Benefit.
